PC PS3 Karliah may become stuck inside Snow Veil Sanctum. After the cut-scene where Mercer stabs the Dragonborn, the camera will be zoomed to first person and stuck there.

Where is the key for Snow Veil Sanctum?
You can find it by traveling South East of Winterhold and North Northeast of Windhelm. The Main entrance is locked and requires a key to open it. The door is unlocked during the quest Speaking With Silence.
Can you marry Karliah without mods?
Karliah is marriable and can be a follower. Compatible with Hearthfire but doesn’t require it. Then go to Nightingale Hall and speak with Karliah. She should have follower lines right away, for marriage lines you must be wearing an Amulet of Mara, like with any other regular candidate for marriage.

After the quest “Hard Answers”, Karliah told me to meet her at the Ragged Flagon, but it never came up in my journal or even came up as a quest. I go to meet her at the Ragged Flagon and she isn’t there. Tried going back to the Sanctum where we first met and she was there, trapped behind a door.

I found Karliah at the Snow Veil Sanctum, in the room where I got poisoned, stuck at the door at the top of the stairs. Just open the door exit and this should solve the problem. Wait a couple of in-game days for her in The Ragged Flagon. Wait in 24-hour intervals, and see if she shows up.
